To retrieve your password you must provide following
information:

1. Choose Entity Type, then Program.
|
|
2. One of the following information: your login name, or License #.
|
|
3. When entering your Certification #/License #, you must enter it in this format: prefix, a dash, and the number.
|
|
4. The email address registered with your online
account.
